Assessment against industry consensus standards, including ANSI, ASTM, and NFPA, where they define accepted practice. We identify the standards relevant to the work activity and evaluate whether the practices and conditions were consistent with those standards.

Review of an employer’s written safety programs, policies, and training records. We assess whether the program was adequate and whether workers were trained for the task and hazard involved.

Evaluation of how workplace hazards were identified and communicated to workers, including labeling, safety data sheets, and warnings. We assess whether the communication met the standard and reached the people at risk.

Analysis of whether the correct protective equipment was required, provided, and used for the task. We evaluate PPE selection, condition, and the adequacy of related policies.

Investigation of energy-control procedures used to protect workers during service and maintenance. We determine whether hazardous energy was properly isolated and whether procedures were in place and followed.

Evaluation of fall hazards and the systems meant to control them, including guardrails, harnesses, and anchorage. We assess whether protection was required, adequate, and in use at the time of a fall.

Analysis of permit-required confined-space conditions and entry procedures. We evaluate atmospheric testing, ventilation, attendant duties, and rescue provisions against the applicable standard.

Investigation of whether machinery was properly guarded to protect operators from moving parts and other hazards. We assess guard design, function, and compliance with recognized safeguarding requirements.

Evaluation of forklift and powered-truck operations, including operator training, vehicle condition, and site conditions. We evaluate how the incident occurred and whether safe-operating requirements were met.

Analysis of crane, hoist, and rigging operations and their role in an incident. We evaluate equipment capacity, rigging practices, inspection, and operator qualification.

Investigation of scaffold and ladder use, including erection, capacity, and condition. We determine whether the access equipment met applicable standards and how it contributed to a fall or collapse.

Investigation of struck-by and caught-between injuries, among the most common causes of worksite harm. We evaluate how the incident occurred and the hazard controls that were, or should have been, in place.

Analysis of workplace electrical hazards, including energized work, grounding, and arc-flash exposure. We assess whether safe-work practices and protective measures met the applicable standards.

Evaluation of safety responsibility on sites where multiple employers work together. We assess the duties of owners, general contractors, and subcontractors, and how those roles relate to an incident.

Investigation of how a workplace incident occurred and the factors that contributed to it. We evaluate the sequence of events, worksite conditions, safety practices, equipment, and work actions to determine how they relate to the incident.

Evaluation of commercial motor carrier operations against Federal Motor Carrier Safety Administration regulations. We assess whether a carrier’s practices met the applicable federal requirements.

Review of driver qualification files and hours-of-service records. We evaluate whether a driver was properly qualified, whether hours-of-service requirements were met, and whether identified compliance issues are relevant to the incident.

Assessment of a fleet’s safety program, policies, and oversight. We evaluate hiring, training, monitoring, and maintenance practices and their role in a crash or loss.

Analysis of commercial-vehicle maintenance and inspection records and their bearing on an incident. We determine whether mechanical condition or deferred maintenance contributed to a failure.
